What is Evidence of Insurability?
Meritain, an Aetna Company, continues to be our exclusive medical healthcare provider for the 2025-2026 benefits year. The Diocese offers employees two medical plans which are open access and do not require you to select a Primary Care Physician (PCP) or obtain a referral to seek care from contracted specialists. All benefit-enrolled employees now have access to 98point6, an on-demand health care service that connects you with a doctor through their mobile app.
Not feeling well? Instead of paying for an office visit, connect with a board-certified physician using the 98point6 app. At no cost to benefit-enrolled employees on the standard plan or high deductible health plan, you can receive a prescription or a treatment plan from a licensed and certified physician.
The Diocese continues to offer to its employees a Dental Premier PPO plan through Delta Dental during the 2025 - 2026 Plan Year. The PPO plan provides both in and out of network coverage. However, you will receive the maximum level of benefits when you use a preferred dentist. VSP will continue to be our Voluntary Vision carrier for the 2025 - 2026 Plan Year. The vision plan provides both in-network and out-of-network benefits. However, you will receive maximum benefits when you use a provider within the network. When am I Eligible?
Full-time Employees: 21 years of age; No service time requirement
Part-time Employees: 21 years of age; 1 year (12 months) of service time with 1,000 hours worked
When can I join?
Full-time Employees: Entry date on the 1st day of the Quarter following hire date
Part-time Employees: Entry date on the 1st day of the next month following eligibility
Is there a match offered by my Employer?
Employer will match up to 6% of pay
Less than 6 years = 50% match
6-10 years = 100% match
11-15 years = 150% match
16-20 years = 200% match
21+ years = 250% match
When am I eligible for the Employer match?

Who owns the money in my account?
Employer contributions vest over a 4-year period, 25% per year.

Medicare Part D
The Medicare Part D notice prescription drug coverage provides notice that open enrollment for Medicare Part D begins October 15th through December 7th. It also states that the Diocese provides a medical plan that on average pays out at least as much as Medicare Part D therefore the Diocesan plan provides enrolled employees credible coverage.CVS/Caremark Prescription Drugs
We use CVS pharmacies and their in-network affiliates for the greatest discounts on your prescriptions. For Specialty Drug Assistance you will use CVS/Caremark and/or Prudent Rx and can contact their Customer Care unit at 1-888-769-9030 with any questions you may have.

This site provides coupons and discounts on prescription drugs. You do not have to be on the Diocesan Plan to take advantage of these discounts. You don’t even need to register and there are no obligations.
Benefit-eligible new employees have a waiting period before benefits may begin.
The waiting period: There is a 30-day waiting period after full time employment begins before you will be eligible to enroll in a Healthcare plan. Benefits would begin the first of the month following 30 days of employment.
Additional Life Insurance:
If you and/or your spouse want Supplemental (voluntary) life insurance coverage above the $180,000 guarantee issue amount for employee coverage and/or above $50,000 guarantee issue amount for a spouse, you must fill out the Mutual of Omaha Evidence of Insurability(EOI) form, print it and mail it to Mutual of Omaha at the following address:
